Facility Evaluation Report
On 10/23/25, Licensing Program Analysts (LPAs) L. Salazar and M. Vega arrived at the facility unannounced to conduct a case management based on incident reports received. LPAs were greeted by staff, stated the purpose of the visit and were allowed entry into the facility.
LPA interviewed Resident R1, Resident R2 and Resident R3. LPA reviewed R1's Centrally Stored Medication records, physician's reports, needs and service plan and services provided. LPAs requested status of R2's eviction notice issued in July 2025. Facility has completed the transition planning meeting. R2 will be relocating to a new facility as on 11/03/25.
Exit interview conducted, no deficiencies cited on todays visit.
