Facility Evaluation Report
On 5/15/25 Licensing Program Analyst (LPA) M. Garza arrived unannounced for an annual inspection visit. LPA contacted Administrator, Vanessa Gardunio and advised LPA was at the facility. LPA met with Administrator a short time later, explained reason for visit and was permitted entry into the facility. Clients were at day program during time of visit.
Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced on 12/03/24. Last fire drill conducted on 5/2/25. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closet in hallway. Sharps, chemicals and medications were located in locked closet. LPA observed sufficient shade and seating in back yard.
The following issues were observed during time of visit: LPA observed freezer in laundry room that was not functioning and in need of repair/removal. Door trim on medication closet missing and in need of patching/repair. Water leaking in bathroom near toilet from wall in need of repair. Spider webs observed throughout yard in need of cleaning. Patio in backyard observed with moss in need of cleaning. South side gate observed with broken board in need of replacement.
LPA requested the following documents to be submitted to CCL by 5/30/25: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-D), Affidavit regarding Resident Cash Resources (LIC 400), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file.
