Facility Evaluation Report
On 12/23/22 at 11:01 PM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct an Annual inspection. LPA was greeted by Licensee/Administrator (LIC) Carrie Blue and given Covid screening. Facility uses two main entry points. Clients, staff, and visitors enter through front facility door. Staff may enter through back facility door. Both have screening set-ups.
LPA toured inside the facility and was observed clean and without any obstructions or fire clearance issues. Facility does not have an outdoor activity area. Hand sanitizer was readily available to residents and visitors. Social distancing and masking is optional in the common areas. Cleaning and PPE supplies were checked.

Exit interview conducted. A copy of this report was given to Licensee Carrie Blue, whose signature confirms receipt of this report.
