The inspector’s account

On 06/29/26, Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct the required annual inspection. LPA was greeted by Program Director (PD) Damion Leon, stated the purpose of the visit and was allowed entry into the facility. LPA met with PD for the annual review.

LPA toured the facility inside and out with PD. LPA observed 4 classrooms. Facility has five, single toilet bathrooms that are gender neutral. Required hand washing signs were observed in the bathrooms. Facility temperature measured at 72 degrees F. Water temperature measured at 109.9 degrees F. Disinfectants and cleaning supplies were observed to be locked in a shed outside. First Aid kits were observed throughout the facility.

LPA observed a kitchenette area in the open office. No lunch is prepared on the premises, clients bring lunch from home and eat in their classrooms. Medications are not dispensed at the day program. There are no clients with restricted health conditions. Cleaning supplies were observed to be locked in shed in the parking lot.

Emergency disaster plan and procedures are in place. Last emergency disaster was earthquake, conducted on 06/1/26. Doors and passageways were observed to be free from obstruction throughout the program. Fire extinguishers were observed with at purchase date of 6/29/26. Samples of client files were reviewed and observed to have the required documents. Samples of staff files were requested and reviewed to have all of the required documents. An updated Infection Control plan was observed. A report of the inspection of the smoke detectors, carbon monoxide and fire sprinklers were documented on the annual basis of 10/22/25

No deficiencies issued during this inspection.

Exit Interview conducted. LPA is requesting the following documents be submitted to the Fresno CCL office by 7/13/26: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E), Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020)
