On 05/15/25, Licensing Program Analysts' (LPAs) L. Salazar and S. Hurt arrived to the facility unannounced to deliver the annual reports from 05/13/25. LPAs were greeted by Program Director (PD), stated the purpose and were allowed entry into the facility.
LPA requested and obtained the facility roster for current clients enrolled/in care at the day program. LPAs discussed the program design and the need for clarification (page 10 - 11) regarding the facility training and the community based training. PD will submit updated design by 05/30/25.
