Facility Evaluation Report
On 05/13/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required annual inspection. LPA was greeted by Program Director (PD), stated the purpose of the visit and was allowed entry into the facility. LPA met with PD for the annual review.
LPA toured the facility inside and out with PD. LPA observed 4 classrooms. Facility has five, single toilet bathrooms that are gender neutral. Required hand washing signs were observed in the bathrooms. Facility temperature measured at 72 degrees F. Water temperature measured at 105 degrees F. Disinfectants and cleaning supplies were observed to be locked in a shed outside. First Aid kits were observed throughout the facility.
LPA observed a kitchenette area in the open office. No lunch is prepared on the premises, clients bring lunch from home and eat in their classrooms. Medications are not dispensed at the day program. There are no clients with restricted health conditions. Cleaning supplies were observed to be locked in shed in the parking lot. First aid kit was observed.
Emergency disaster plan and procedures are in place. Last emergency disaster was earthquake, conducted on 04/09/25. Doors and passageways were observed to be free from obstruction throughout the program. Fire extinguishers were observed to be charged. A sample of client files were reviewed and observed to have the required documents. Staff files were requested and required documentation will be sent next day. An updated Infection Control plan was observed.
LPA requested the following updated forms to be faxed to CCLD by 05/23/25: Designation of Facility Responsibility (LIC308), Administrative Organization (LIC309), Client Roster (LIC 9020) and Emergency Disaster Plan (LIC 610D).
