COMPLAINT CONTROL NUMBER: 24-AS-20240201112443

Facility did not follow client’s prescribed diet resulting in injury and hospitalization

On 05/07/24, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to deliver findings on the above allegation. LPA was greeted by Program Director, stated the purpose of the visit and was allowed entry into the facility.

The Department has investigated the above allegation. Based on interviews and records review, although the allegation may have happened, there is not a preponderance of evidence to prove that the alleged violation occurred, therefore the allegation is unsubstantiated. Exit interview conducted and a copy of this report was provided at the time of visit.
