Staff spoke inappropriately to client in care.
On 9/8/23 at 12:09 PM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct a complaint investigation. LPA explained reason for inspection and met with Administrator (ADM) Lindsey Devries.
LPA reviewed records and conducted interviews. Based on record reviews and interviews conducted, LPA found that staff spoke inappropriately to client in care. During the facility's internal investigation, S1 admitted to facility management that S1 was shaking the shoe and using an aggressive tone stating to C1 "you're going to get in trouble for this". S1 was terminated two days following the incident. The “preponderance of the evidence” standard has been met. The above allegation is substantiated.
A deficiency is being cited based on LPA record review and interviews conducted in accordance with the California Code of Regulations, Title 22, see LIC9099D. Exit interview was conducted and a Plan of Correction was reviewed and developed with the Administrator. A copy of this report and appeal rights were discussed and left with Administrator Lindsey Devries, whose signature on this form confirms receipt of these documents.
