Facility Evaluation Report
On 11/12/24, Licensing Program Analysts (LPA) L. Salazar and M. Medina arrived to the facility unannounced to conduct the required annual inspection. LPAs were greeted by the receptionist, stated the purpose of the visit and were allowed entry into the facility.
LPAs met with Regional Director and Program Director. LPA Medina conducted a review of Staff and client records and documented on a separate report. LPA Salazar conducted the facility tour with Regional Director (RD) and Program Director, Kylie Gonzales. No fire hazards or passageway obstructions were observed. There are no bodies of water on the premises. Fire extinguishers were observed with a service date of 06/24/2024. Emergency Disaster plan was observed to be updated and complete. Emergency Disaster Drill logs were observed for all staff. Last drill was an earthquake drill conducted 10/17/24. Facility temperature was 69 degrees F.
LPA observed 6 classrooms (Sensory, Computer, Gym, Game Room, Library and Art Room) and 3 bathrooms, (total of 4 toilets). Classrooms were observed to have sufficient seating and adequate lighting. LPA observed restrooms to be clean, have operational lights, running water and are free from odor. Hot water measured at 119 degrees F.
LPA observed a kitchen/dining area where clients participate in group activities and have lunch. No lunch is prepared on the premises, clients bring lunch from home. Medications are not dispensed at the day program. Cleaning supplies were observed to be locked in a room off of the hallway. First aid kit was observed and contained all required items.
An exit interview was conducted with Regional Director. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit.
