The inspector’s account

On 07/10/25, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct a case management visit based on incident reporting. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. Administrator was called and arrived with District Manager shortly after LPAs arrival.

The Department received an incident report for Resident R1 for the date of 06/13/25. LPA reviewed and obtained medical records dated 06/19/25 from R1's physician. Records show an x-ray of the neck was taken and R1 was also tested for strep throat. A referral has been made for psychiatry. R1 has a follow up visit with their psychologist on 07/22/25. Should this incident occur again, Administrator has a plan in place and has scheduled an in-service training for staff members to seek immediate medical attention should R1 present this behavior again.

Responsible party for R1 was notified the same day as incident and attended R1's medical appointment. LPA reviewed and observed emergency intervention training certifications and CPR/First Aid certifications for Staff S1, Staff S2 and Staff S3. Certifications were current.

No deficiencies cite on today's visit. An exit interview was conducted with Administrator and District Manager. A copy of this report was provided to Administrator.
