The inspector’s account

On 03/24/25, Licensing Program Analysts L. Salazar and M. Garza arrived to the facility unannounced to conduct a case management, health and safety check visit based on self reported incident. Reports submitted by the Administrator in regard to Resident R1's condition. LPAs were greeted by Administrator, Gladys Hernandez and Quality Assurance Specialist, Dee Rose, stated the purpose of the visit and were allowed entry into the facility.

On 12/02/24, R1 was taken to Urgent Care as a follow up to an ER visit on 11/29/24 for an eye infection and a non-healing self inflicted wound, R1 continues to pick at, continually rubbing their back towards their lower neck making it raw. Medical attention was sought and ointment was prescribed and a plan a care was provided. R1 was referred to Home Health for wound care treatment.

On 12/18/24, R1 was taken back to Urgent Care to conduct a swab test on the self inflicted would to verify it was not a contagious infection. On 12/19/24, R1 was again seen by physician and was prescribed ointment, swab tests returned negative for contagious infection.

On 02/05/25, it was reported there was a change in condition in R1. R1 was observed to have red and swollen skin and was taken to ER. R1 was diagnosed with cellulitis and was prescribed antibiotics. On 02/07/25, R1 had increased behaviors and agitation and was taken to the Emergency Room. On 02/17/25 R1 was again taken to the ER via ambulance due to altered mental status. On 02/21/25, R1 was admitted to the hospital and has not returned to the facility. Per physician's reports, R1 required a nursing care/Skilled Nursing facility.

No deficiencies cited during today's visit. Exit interview conducted with Administrator, Gladys and Quality Assurance Specialist, Dee. A copy of the report will be provided via email within 24 hours.
