Facility Evaluation Report
On 08/13/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct a case management visit based on a Decision and Order received. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. LPA met with Mariana Alba Viayra.
LPA met with Administrator who stated, Staff 1 (S1) was never hired and never worked for the facility. LPA reviewed current facility staff schedule and facility's fingerprint clearance associations. LPA did not observe S1 to be employed or present in the facility. The Guardian Background check system does not show S1 as associated.
An exit interview was conducted and a copy of this report was provided. No deficiencies cited.
