Facility Evaluation Report
On 12/09/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an attempted required Annual visit. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee Lue Jean who granted LPA entry into the facility. LPA met with Licensee Lue Jean Loring. There are currently no clients who reside at the facility. LPA observed staff Elijah Jones and individual Michael Jones present in the common area.
Shortly, Michael Jones informed LPA to leave the facility per telephone call to Carlton Jones. LPA stepped out to make telephone calls with Regional Manager B. White and Licensing Program Manager A. Walton. LPA returned to facility shortly at 11:52 AM, staff Elijah Jones refused LPA entry into the facility. LPA left business card with date and time with staff Elijah Jones.
Annual inspection was not conducted. The department will return at a later date to conduct the Annual Inspection.
