The inspector’s account

On 10/04/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection on this date. LPA introduced self and stated purpose, LPA allowed entrance by Licensee/Administrator, Marycruz Leyva contacted by telephone and arrived a short time later to conduct visit.

Residents observed to be relaxing in the living watching television and others engaging with staff at time of visit.

Facility toured inside and outside. Facility observed to be clean and a comfortable temperature. All common areas have adequate seating and lighting available for residents in care. Resident bedrooms toured and observed to have all required furnishings available. Linen observed to to clean and in good repair. Resident bathroom toured. All bathrooms fixtures observed to be in good repair and operational. Bathroom observed to have shower chairs, skid resistant mats and grab bars. Hot water measured 107 degrees F. Medications observed to be stored and secured in kitchen cabinet. Medications reviewed to and observed to have original labels and be administered as prescribed.

The fire extinguisher was serviced on 01/16/2025. Carbon monoxide and smoke detectors tested and observed operational during inspection. Facility is equipped with a pull station and fire sprinkler system. The last fire drill was conducted on 8/02/2025 according to facility records.
