The inspector’s account

On 07/27/2026, Licensing Program Analysts (LPAs) M. Medina and J. Duarte conducted an unannounced Annual Required Inspection. LPAs introduced self, stated the purpose of the visit and allowed entrance to Facility. LPAs conducted facility tour with Administrator, David Cervantes and Residential Facility Manager, Javier Hernandez.

LPAs conducted a tour inside and outside of facility. Facility tour began in occupied in Building #7, building observed to be a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. LPAs toured Room #1, #4, #7, #11 and #13. Resident bedrooms observed to have required furnishings, all resident bedrooms have private bathrooms. LPAs observed water temperature in the bathrooms toured to measure between 108 degrees to 114 degrees F. LPAs observed the bedrooms and bathrooms toured in need of cleaning. Kitchen toured, facility receives weekly food delivery from US Foods. Facility observed to have a 2-day perishable food and 7-day of non-perishable food available. Menu observed to be posted near kitchen area. LPAs observed multiple areas through out facility with dead insects that need to be cleaned.

Exterior tour conducted, all exits open and free of obstructions. Medications observed to be locked and secured in medication room, medications observed to have original labels and be administered as prescribed.

Fire extinguishers were last serviced on 12/09/2025 . Facility is equipped with fire sprinklers and pull stations. Fire alarm and Carbon monoxide detectors observed to be functional during today's inspection. All cleaning supplies and toiletries observed to be locked and secured in laundry room. Last fire drill was conducted on 6/19/2026 during today's inspection.

LPAs reviewed resident files. Staff files will be sent to Fresno Regional office via fax

LPA Medina received copies of LIC 500, LIC 9020, LIC 610D and Infection Control Plan during visit..
