Facility Evaluation Report
On 3/17/2026, Licensing Program Analysts (LPAs) M. Medina and K. Brown conducted an unannounced Case Management visit for the purpose of conducting a health and safety check of residents in care.. LPAs arrived, stated purpose of visit and met with Administrator, David Cervantes to conduct visit.
LPAs toured facility with Administrator. Currently, there are 9 residents in care, residents observed to be in common areas and resting in their bedrooms. Facility observed to be well lit, a comfortable temperature, and clean. Kitchen toured, facility has adequate food supply for residents in care.
During facility visit, LPA provided Administrator with updated facility license to be posted at entry. LPA requested and received a copy of current resident roster and staff roster.
