10/04/2023 and conducted by Evaluator Melinda Medina

COMPLAINT CONTROL NUMBER: 24-AS-20231004145833

Lack of supervision resulted in a resident walking nude on a public street.

On 11/30/2023, Licensing Program Analyst (LPA) M. Medina arrived to conduct a subsequent complaint visit and deliver findings. LPA Medina met with Administrator, Javier Hernandez and stated purpose of visit.

During course of the investigation, LPA conducted interview with Administrator. The department had insufficient information regarding allegation that lack of supervision resulted in a resident walking nude on a public street occurred. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or disprove that the allegation occurred therefore the allegation is unsubstantiated.
