The inspector’s account

On 7/21/2026, Licensing Program Analyst (LPA) M. Medina arrived to conduct an unannounced Annual Required inspection. LPA arrived, stated purpose of visit and allowed entrance by direct care staff. Licensee/Administrator Elysia Boyd was present and conducted visit with LPA

Currently, there are six (6) residents in placement. Residents were present during today's inspection and observed to be having lunch at the dining room table at the time of LPA arrival.

Facility observed to be well lit, clean, odor free and a comfortable temperature. LPA observed, dining room, living room and family room to have adequate seating and lighting for residents. Resident bedrooms observed to have all required furnishings. Resident bathrooms toured, all fixtures observed to be operational. Water temperature measured at 113 degrees F. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day of non-perishable food available, facility also observed to have an emergency food supply available. All knives observed to be locked and secured in kitchen cabinet. Medications observed to be locked and secured in kitchen cabinet. Medications observed to have original labels and to be administered as prescribed.

Carbon monoxide detectors observed operational during inspection. Facility is equipped with pull stations and fire sprinkler system which is monitored by Jorgensen Company. Fire extinguisher present with a service date of 1/05/2026.

Outside of facility toured. LPA observed all exits to open free of obstruction. LPA observed shade available on back patio with a table and chairs for residents.

No deficiencies observed during today's inspection. Exit interview conducted with Administrator and a copy of report provided for facility records.
