Facility Evaluation Report
On 7/15/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA arrived, introduced self and allowed entrance by House Manager, Blanca Lucio. Licensee Elysia Boyd was unavailable to conduct today's inspection. Facility RN, Donna Davis was also present to conduct facility visit with LPA.
Facility currently has four (4) residents, all residents were present during today's inspection. Residents observed interacting with staff. Facility appears clean, odor free, and at a comfortable temperature. Sufficient lighting and seating in both dining room and living room to accommodate all residents. Resident bedrooms observed to have required furnishings . Linen is sufficient and in good repair. LPA observed resident bathrooms to have shower chairs and grab bars available. Water temperature measured at 120 degrees F. LPA observed facility to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Knives observed locked and secured in kitchen cabinet. All medication observed to locked and stored in cabinet. Medications observed to have original labels and to be administered as prescribed.
Fire extinguishers have a service date of 1/05/2026. LPA observed carbon monoxide detectors and smoke detectors to be operational during inspection. Last fire drill conducted on 7/6/2026 according to facility records.
Outside of facility toured, all fire exits open freely and observed to free of obstruction. LPA observed pool to be surrounded by a locked gate and inaccessible to residents.
Resident and staff files reviewed during inspection. LPA received copies of current administrator certificate and CPR/First Aid card during inspection.
