Facility Evaluation Report

On 8/03/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA contacted Licensee by telephone who arrived a short time later to conduct visit with LPA.

LPA toured the facility with Licensee. Facility is a three bedroom (3), three and half (3 1/2) bathroom home. There are 3 bedrooms and 2 1/2 bathrooms downstairs and a master suite and bathroom upstairs.

Currently, there is one (1) resident in placement. Resident was not present at time of inspection. Facility observed to be well lit and a comfortable temperature. All common areas observed to have adequate seating available. Resident bedrooms toured, all bedrooms observed to have the required furnishings, lighting and bed linens. Kitchen observed to have a 2-day supply of perishable food and a 7 day of non-perishable food available. Knives are kept in a locked cabinet.

Cleaning supplies and chemicals are stored in a locked cabinet in laundry room. Fire extinguisher present with a service date of 8/04/2026. Carbon monoxide and smoke detectors were present and observed operational at time of the inspection.

Outside of the facility toured. There is a large shaded patio with seating available for resident. All exits open free of obstruction. No hazards observed.

No deficiencies cited. A copy of report provided to licensee for their records.
