Facility Evaluation Report
On 07/13/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection visit. LPA arrived and contacted Licensee/Administrator Diana Rico by telephone who arrived a short time later to conduct visit with LPA.
Currently, there are four (4) residents in care, all residents were at day program at time of inspection. LPA tour began in kitchen, LPA observed adequate supply of plates, cups and utensils for residents. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Dining room and living room observed to have adequate seating and lighting available for all residents in care. Resident bedrooms toured, all bedrooms observed to have required furnishings and have adequate lighting. Resident bathrooms toured, all fixtures observed operational. Water temperature measured at 119 degrees F. Medications observed to be locked and secured in cabinet near staff office.
All chemicals observed to be locked and secured in a cabinet in the garage. Fire extinguisher present with a service date of 4/08/2026 . Smoke detectors and carbon monoxide detectors observed operational at time of inspection. Last fire drill was conducted 6/05/2026 according to facility records.
Outside of facility toured. All exits open free of obstruction. No hazards observed. Facility has seating and shade available for residents in care.
Staff and resident files reviewed. LPA requested a copy of facility surety bond, administrator certificate, LIC 500 and LIC 9020 to be submitted to Fresno Regional office no later than Friday, July 31, 2026.
No deficiencies observed during inspection.
Exit interview conducted. A copy of signed report provided to Administrator for facility for records.
