Facility Evaluation Report

On this date, Licensing Program Analyst (LPA) M. Yang arrived to conduct an unannounced initial complaint investigation and met with Diana Rico, Administrator. Four resident were present during tour. LPA toured the facility, interviewed clients and staffs. During the course of the investigation, LPA observed a bottle of bleach in an unlocked cabinet.

Deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6.
