Facility Evaluation Report
On 7/13/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit to follow up on a self reported incident submitted to department by Licensee/Administrator, Diana Rico.
On 7/09/2026, resident 1 (R1) was out in the community during unsupervised time attempting to cross the street when they were struck by a vehicle.
R1 contacted staff at facility to advise them of incident. Licensee was notified by staff and arrived on scene to gather additional information. Upon arrival, Licensee observed emergency services on site as well as Porterville Police Department. Parties involved stayed on scene and a case number provided for incident. R1 was transported to emergency room to be evaluated and released 2 hours later with minor scrapes, no visible bruising and possible sprained ankle. R1 was able to ambulate and apply pressure without the use of crutches.
LPA obtained a current copy of IPP dated 2/18/2026 and physician report dated 9/23/2025 of which both document that R1 does have unsupervised time in community and able to leave facility unassisted.
