Facility Evaluation Report
Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today March 10, 2026 for the facility’s annual inspection. LPA introduced themselves and asked to speak with the Administrator. Administrator Marsha Shoemake was contacted and arrived shortly after.
Facility is licensed for 270 clients and with today’s census it is 62. Water temperature was checked in one of the woman's bathrooms and read at 93.8 degrees Fahrenheit and in men’s bathroom which read at 98.2 degrees Fahrenheit. Fire Extinguisher was serviced June 9, 2025 and is within the safety regulation period. Fire system was check 11/21/2025 and passed inspection. Carbon monoxide detectors was tested and in working order.
Marsha Shoemake is not required to have Administrator's Certification, but has proper documentation, schooling, and training. Staff files were reviewed, annual training's were not documented. Sample of resident files were reviewed, complete, and current. Facility provided a log for disaster drills that are conducted monthly. Cleaning supplies are locked and inaccessible. LPA observed scissors being accessible, nail polish remover in an unlocked cabinet, knives were in cabinet with plastic child proof device, crafting bonding glue accessible.
LPA inspected the interior and the exterior of the facility including various classrooms.
Deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
Exit interview conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Administrator Marsha Shoemake.
