Facility Evaluation Report

On 7/21/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management to self reported incident received in the Fresno Regional office on 7/17/2026. LPA arrived, introduced self, stated purpose of visit and allowed entrance to day program.

LPA met with Curtis Poe, Program Supervisor, Carol Ledbetter, Director of Program Services and Pam Gruber, Executive Director to discuss incident.

Per information SOC 341 and incident report received by department involving resident 1 (R1) against staff 1 (S1). The incident was reported to day program staff on 7/16/2026 but allegedly occurred a couple of weeks prior.

During initial reporting to department on 7/16/2026, LPA Medina was informed that S1 was placed on administrative leave pending investigation by Porterville Police Department. During Case Management visit on 7/21/2026, LPA was informed that S1 was terminated on 7/17/26.

During case management visit, LPA received copies of termination paperwork and copy of incident report previously submitted to department on or about 4/07/2026 also involving S1.

LPA provided facility rosters to Program Supervisor and Director of Program Services to disassociate S1 from programs during visit.
