Facility Evaluation Report
On 7/13/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced themselves, stated purpose of visit and allowed entrance. Program Supervisor, Curtis Poe was contacted and conducted inspection with LPA..
Facility toured with Program Director. Day Program observed to be clean, odor free, and a comfortable temperature. LPA toured both classrooms which are set up with individual work stations, day program, art room, sensory room which is also utilized as quiet room. Facility is equipped with a kitchen that has microwaves and a refrigerator available for client use. Clients bring their lunch from home to day program or may purchase meals during community outings. There are no cooking experiences conducted on site. No medications are administered or stored on site. Client restrooms toured, all fixtures observed to be operational at time of inspection. Water temperature measured within regulation at bathroom sink.
Facility extinguishers present in both classrooms and in the kitchen with a service date of 9/19/2025. Facility is equipped with pull stations, strobe lights, sprinklers throughout facility. Last fire drill conducted on 6/19/2026. First Aid kit present and observed to have all required items.
No deficiencies cited. Exit interview conducted and a copy of report provided for day program records.
