On February 23, 2026 Licensing Program Analyst (LPA), B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA knocked at the facility door but there was no answer. LPA attempted to call phone numbers on file, and there was not answer.
LPA will document this visit as an attempt visit and will return at a later time and or date to conduct unannounced visit.
