The inspector’s account

On 04/21/2025, Licensing Program Analysts (LPAs) M. Melinda and J. Duarte conducted an unannounced Annual Required inspection. LPAs were allowed entrance by House Manager, Ruth Ramirez. Also present was Licensee, Kari Gaither.

Currently, there are four residents in placement. All residents were at day program at the time of inspection. Residents attend day program Monday to Friday from 8:00 AM. to 2:00 PM.

LPAs conducted a complete tour of the facility with Staff Ruth. The facility was observed at comfortable temperature (74 F), clean, and in good repair. The tour started with the residents bedrooms. The resident bedrooms were observed to be adequately furnished with a bed, dresser, chair, and adequate lighting. Bathrooms were properly equipped and the fixtures were functioning properly. The water temperature in the hallway was at 107.2 F. The kitchen was toured and sharps were observed in a locked drawer next to the sink. The chemicals were also observed to be locked in the cabinet right below the kitchen sink. An adequate supply of food was observed for residents in care . Common areas were well furnished and well lit. The backyard has a patio with a table and chairs. The outside gate was observed to be functioning and self latching. Exits were free of obstructions.

All medications were observed to be locked in and stored in the entry closet. Medications were observed to have original labels and being administered as prescribed

Fire extinguishers observed in the laundry room and at facility entrance, with a service date of 03/04/2025. Carbon monoxide and smoke detectors were tested and were observed to be operational at time of inspection.

No deficiencies observed. An Exit Interview was conducted. A copy of the report was provided to the licensee for facility records.
