Facility Evaluation Report
On 6/25/2026, , Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA arrived, introduced self, stated purpose of visit, and allowed entrance. LPA met with Connie Villarreal, House Manager to conduct inspection. Administrator, Anna McDonald contacted by telephone and not available to conduct today's inspection. Administrator, Bailey Rackley arrived a short time later to conduct inspection with LPA.
Facility tour conducted inside and outside. Facility is a 3 bedroom, 2 bathroom home, there are 2 shared bedrooms and 1 private bedroom. Facility observed to be clean, odor free, and a comfortable temperature. All resident bedrooms observed to have required furnishings. Resident bathrooms toured, bathrooms observed to have soap and paper towels available. Water temperature measured at 105 degrees F. All fixtures observed to be operational. All common areas observed to have adequate seating available for residents in care. Kitchen toured. All sharps observed to be locked and secured in medication cabinet. Facility has a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Medications observed to be locked and secured in cabinet near kitchen. Medications reviewed and observed to have original labels and to be administered as prescribed. Centrally stored medication log observed.
Fire extinguisher present with a service date of 03/05/2026, last fire drill conducted on 5/14/2026 according to facility records. Smoke detectors and carbon monoxide observed operational at time of inspection.
Outside of facility toured, all exits open free of obstruction. No hazards observed.
No deficiencies cited during inspection. A copy of report provided via e-mail for facility records.
