Facility Evaluation Report

Licensing Program Analyst (LPA) J. Duarte arrived at the facility unannounced to conduct a Case Management - Incident visit. LPA met with and explained the reason for the visit with Administrator Dustin Wall.

The reason for the Case Management is to follow up on a Special Incident Report (SIR) regarding an allegation of physical abuse, staff to resident dated 06/08/2026. The facility reported this incident to CCLD as required.

During this visit, LPA conducted interviews with resident and staff. Record review of resident and staff member was conducted during this visit. interviews revealed that staff treat R1; "nice, R1 gets along with staff, staff take R1 out for doctor appointments and out to eat." The facility also conducted their own investigation and the allegation was unfounded.

There were no citations issued. An exit interview was conducted and a copy of this report was left with the administrator, whose signature confirms receipt of this report.
