12/23/2024 and conducted by Evaluator Mai Yang

COMPLAINT CONTROL NUMBER: 24-AS-20241223134104

On 01/23/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct subsequent complaint investigation and deliver findings on the above allegation. LPA introduce self and stated the purpose of the visit. LPA met with Licensee Lisa Ong.

During the course of the investigation, LPA conducted interviews and reviewed records. Based on interviews conducted and records reviewed, it was confirmed R1 resided at the facility until 01/15/25. Therefore, the allegation above is UNFOUNDED, meaning they were false, could not have happened, and/or are without reasonable basis. We have therefore dismissed the complaint. Exit interview conducted. A copy of this report was provided to Licensee.
