Facility Evaluation Report

“This report is being amended to update the reporting manager.”

LPA was at the above facility for a case management health and safety check visit. I met with Licensee Lisa Ong and informed her the purpose of the visit. Today Tulare County Health & Human Services Agency (TCHHSA) relocated 2 to St. Michaels, 4 to Sierra Valley Rehab.(SV), 1 is at Khaweah Hospital (KH) in Visalia, 1 is in Jail and 2 still need medical assessment so they will remain in the facility until tomorrow. According to TCHHSA, the residents that went to SV, KH and jail will be place in an appropriate home when they are discharged. The 2 residents still at the facility will be relocated to St. Michaels in Dinuba today.
