Facility Evaluation Report

“This report is being amended to update the reporting manager.”

LPA was at the above facility conducting a case management health and safety check. I met with Administrator Tony Ong and Licensee Lisa Ong. The three meal were provided by meals on wheels and snacks were provided by the facility. Tulare County Mental Health team were on the premises providing assessment and help with the relocation of the residents. Today, a total of 14 residents were able to be relocated. Of the remaining 10 residents, 2 are in the hospital/medical clinic/rehab., 1 in jail and 7 are still in the facility awaiting further assessment. Tulare County Mental team will return tomorrow. There were three staff on duty during the visit.
