LPA was at the above facility for a health and safety case management visit. LPA met with Administrator Tony and Licensee Lisa Ong. Tulare County Mental Health Team that came to conduct assessment for the residents just left during my arrival. Diner was delivered and served while I was still on the premises. During a facility tour, I came across resident 1 (R1) who was not accounted for during my previous visit. With the additional resident, the resident currently has 24 residents. There were three staff present during my visit.
