Facility Evaluation Report

“This report is being amended to update the reporting manager.”

LPA was at the above facility to conduct a case management health and safety check. LPA met with on Licensee Lisa Ong and informed her the purpose of the visit. Staffs present at the facility were Administrator, Tony Ong and Ligaya Escario. A tour of the facility were ducted. LPA spoke to licensee and residents that were awake and confirmed breakfast and lunch were delivered by by Meals on Wheals. Dinner was delivered while LPA was still on the premises. LPA informed Lisa Ong that the facility can't use the facility's kitchen to cook food, if they must provide food, they will need to have it catered.
