Facility Evaluation Report

“This report is being amended to update the reporting manager.”

LPA was at the above facility to conduct a case management health and safety check. LPA met with on duty staff L. Escario and informed her the purpose of the visit. A tour of the facility were ducted. The residents from rooms 22 and 18 were relocated to rooms 12 and 21 due to bedbugs and/or bedbug treatment. The residents that were awake and I was able to have a conversation with all indicated they got breakfast and lunch from outside food delivery. They also indi cated that the facility is closing and they will be moving to St. Michael's in Dinuba. L. Escario was the only staff on duty during my visit.
