Facility Evaluation Report
On 06/25/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required visit. LPA introduced self, stated the purpose of the visit, and allowed entrance by Administrator Raymond Taylor.
Currently, there are 4 residents in care. Two residents were present at time of inspection and observed to be relaxing, the other 2 residents were in the community. The facility was observed to be at a comfortable temperature, clean, and free of odor. LPA observed adequate seating available for all residents in common area. Resident bedrooms toured, LPA observed resident's bedroom to have required furnishing available. Bathrooms observed to have grab bars, shower chairs, and slip resistant mats available. Bathroom fixtures observes operational, hot water temperature was tested at 112 degrees F. Kitchen toured, LPA observed the sharps and knives to be locked and secured in kitchen drawer. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Medications are locked and secured in hallway closet. Fire extinguisher present with a service date of 4/17/2026, carbon monoxide and smoke detectors tested and observed operational at time of inspections.
Outside of facility toured. Side gate was self-closing and self-latching. Facility has large shaded patio with outdoor seatings available for residents. All exits observed to open freely and are free of obstruction. Pool is surrounded by a fence that is locked and secured and inaccessible to residents.
LPA reviewed resident and staff files during inspection. LPA received copies of current administrator certificate and liability insurance during inspection.
No deficiencies cited during inspection. Exit interview conducted and a copy of report provided to Administrator for facility records.
