Facility Evaluation Report
Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Dessirae Pinheiro.
Facility is licensed for 6 residents and has a current census of 4. Water temperature was checked in the kitchen which read at 108.5 degrees Fahrenheit. Fire Extinguisher was purchased January 30, 2026 and is within the safety regulation period. Smoke and carbon monoxide detectors were tested and in working order.
Christopher Agda’s Administrator Certification expired on October 9, 2025. Christopher is currently working on his current Administrator Certification. Staff files were reviewed, are not complete, and there is no current training on file except first aid. Resident files were reviewed, complete, and current. Facility provided a log for disaster drills that is not current. Toxins and cleaning supplies are locked and inaccessible. There is a locked storage for medications. LPA observed knives to unlocked and accessible to residents in care.
LPA inspected the interior and the exterior of the facility including the common living space, resident bedrooms, bathrooms, medication storage, kitchen, garage and outdoor area. LPA observed R1 to have a worn mattress which caves in the middle of the bed, R2's fitted sheet needs to be washed. LPA observed common bathroom to have a heavy urine odor, mildew build up in the shower, and mold inside of the toilet. Bedrooms were furnished with adequate lighting. Food supply is adequate for 2-day perishable and 7-day nonperishable.
Facility did not have a current infection control plan or disaster plan on file.
