Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct a Case Management for a Health and Safety check for an incident that occurred on 12/18/25. LPA contacted Administrator Kyle Wolden via telephone who gave permission for Staff Rayne Marshall to sign for this report. .
LPA toured the facility. Facility was clean. Facility has an7 day non perishable and 2 perishable food supply. Client file was emailed to LPA. Administrator advised clients were attending a Christmas party during the visit.
