Facility Evaluation Report
On 06/03/26, Licensing Program Analyst (LPA) M. Yang, arrived unannounced to conduct an Annual Required Inspection. LPA introduced self and stated that purpose of the visit. LPA met with Administrator Curtis Poe and Program Coordinator Stephanie Arredondo.
LPA toured inside and outside of the facility. No fire hazards or passageway obstructions were observed. LPA observed sufficient seating and adequate lighting throughout the facility. Clients provide their own meals. Knives kept locked in counter drawers. Facility does not administer medications to clients. LPA observed chemicals locked in a storage cabinet. Fire extinguishers were last serviced on 09/23/25. Last fire drill completed on 05/21/26. LPA toured the client restrooms which appeared clean and operational. Hot water measured at 107.7 and 108.1 degrees F in women bathroom and 107.5 degrees F in men bathroom. Outside of facility was toured. Sample of staff and clients’ files were reviewed.
Exit Interview conducted. LPA received copies of current Lic 308, Lic 500, Lic 610D, and client rooster during visit. A copy of this report was provided to the Administrator, whose signature confirms receipt of this report.
