The inspector’s account

On 06/03/26 Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit and was greeted by Licensee Dennis Morris and staff Norma Hernandez. LPA was granted entry and met Administrator (A1) Denisse Yanez. LPA toured facility with L1 and A1. Two clients were present during inspection. Two clients arrived later during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Fire extinguisher was observed with purchased date 11/07/25. Fire drill last completed: 04/10/2026. First Aid observed with required items. An adequate supply of perishable and non-perishable food was observed. Extra linens and towels were observed in hall cabinet. Medications observed were locked in living room cabinet. Cleaning supplies were kept chemicals stored and locked under kitchen sink, under kitchen counter, and laundry shelves. All bedrooms were observed to have the required furnishings and adequate lighting. The bathroom was toured. Hot water temperature was tested at 113.4 degrees F in bathroom. Tools were observed locked in the garage. Outside of the facility toured and observed to be free of debris. Adequate outdoor seating is available for clients. Swimming pool observed locked and inaccessible to clients. Side gate observed self-closing and self- latching. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ and sample of staff files were reviewed to have all the required documents. Client’s P & I was checked. Medications were checked and MARs were reviewed.

Exit Interview conducted. LPA received copies of Lic 308, Lic 400, Lic 402, Lic 500, Lic 610D during visit. A copy of this report was provided to Licensee, whose signature on this form confirms receipt of this report.
