Facility Evaluation Report
05/05/2026 09:20 AM Licensing Program Analyst Rebecca Knight conducted an unannounced case management visit and met with licensee Lauren Leak. Today’s visit is regarding an incident that occurred on 04/10/2026 and was reported to licensing the same day.
It was reported that on 04/10/2026 06:52 AM a family member of a client who lives in the home had arrived at the facility to bring the client back to the facility and found that no staff member was present in the facility. The family member stated that Staff 1 (S1) returned to the facility a few minutes later. The licensee reviewed exterior security camera footage and reports that S1 was seen walking to their vehicle at approximately 6:52 AM The client's family member arrived at the facility at approximately 6:54 AM. At approximately 06:57 AM S1 returned to the facility. There were no other staff in the facility during this time therefore all clients were left unattended for 5 minutes, which constitutes absence of supervision. S1 was terminated from employment as a result.
Absence of supervision is a zero-tolerance violation. As a result, an immediate civil penalty in the amount of $500.00 is being assessed this day and is documented on the attached LIC421IM.
Based on interviews and evidence obtained during the investigation, the preponderance of evidence standard has been met, therefore, the allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22), is being cited on the attached LIC9099D. Appeal rights were provided. Exit interview conducted and a copy of the report was provided to licensee Lauren Leak.
