Facility Evaluation Report

On March 28, 2025, at 1:30 p.m. the Department held an office meeting to discuss an incident that had occurred at the facility. Present at this meeting were Community Care Licensing representatives, Alycia Rayner, Regional Manager (RM), Lauren Crocker, Licensing Program Manager, and Kayla Adkison, Licensing Program Analyst, facility representatives, Tamra Leak and Lauren Leak, and Far Northern Regional Center representatives Corey Oller and Daphne Wood.

All representatives at this meeting concur that the resident in question is not appropriate for this facility based on the change in condition since admittance that has resulted in the need for a higher level of care. The Far Northern Regional Center will submit a plan for alternate placement for this resident.

RM Rayner has requested a follow-up meeting for next week for an update on the situation.
