Facility Evaluation Report
08/19/2025 11:45 AM Licensing Program Analyst Rebecca Knight conducted an unannounced case management visit and met with site supervisor Maria Mata. The purpose of the visit was to follow up on citations that were issued during the 2025 annual inspection.
LPA toured the facility with Ms. Mata to confirm that the following physical plant deficiencies have been addressed and completed.
Air conditioning non-functional in administrative building. LPA confirmed that the air conditioning is now functioning.
Ceiling fans inoperable in staff conference room in administrative building. LPA confirmed that ceiling fans are functioning.
Window screen missing from 1 of 4 bathrooms in multi-purpose room / common area. LPA confirmed that window screen is in place.
Soap dispenser missing from 1 of 4 client bathrooms in multi-purpose room / common area. LPA confirmed that soap dispenser is in place.
No deficiencies were cited as a result of the inspection. A copy of the report was provided to administrator Esperanza Chavez.
