arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with administrator Esperanza Chavez and explained the purpose of the visit.

LPA Knight and the administrator toured the facility together to ensure the health and safety of clients who attend program. Areas toured include but are not limited to common areas in two buildings, bathrooms, kitchen, storage areas and patio area. Staff and resident files were reviewed. All employees requiring background checks are cleared.

Kitchen was clean and in good repair. Fire extinguishers fully charged and inspected. Last disaster drill was conducted in July 2025 which was an earthquake drill, the facility has been conducting fire drills monthly.

LPA toured the facility and observed the following physical plant violations:

The facility was observed to be at a comfortable temperature in the clients common area / activity room, air conditioning is fully functioning. LPA observed that the central air conditioning unit was not functioning in the entire administrative building. LPA observed fans in the administrative building which are inadequate to cool the building for an entire day. On the day of the inspection the administrative building temperature measured 80 degrees and the outside temperature was 86 degrees. Administrator confirmed that they have a work order in to repair the air conditioning, a part is on order. The technicians are scheduled to come and repair the air conditioning unit on 07/31/2025.
