Facility Evaluation Report
07/29/2025 10:00 AM Licensing Program Analyst (LPA) Rebecca Knight arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with administrator Esperanza Chavez and explained the purpose of the visit.
LPA Knight and the administrator toured the facility together to ensure the health and safety of clients who attend program. Areas toured include but are not limited to common areas in two buildings, bathrooms, kitchen, storage areas and patio area. Staff and resident files were reviewed. All employees requiring background checks are cleared.
The facility was observed to be at a comfortable temperature. Common areas, kitchen and bathrooms were generally clean and in good repair. Fire extinguishers fully charged and inspected. The fire marshal comes in the Fall yearly to inspect the smoke alarms, fire extinguishers and carbon monoxide detectors. Last disaster drill was conducted in June 2026 which was an explosion drill, the facility has been conducting fire drills monthly.
During today's inspection LPA observed the following physical plant violations:
Exhaust fans inoperable in 2 of 4 bathrooms located in the main activity building.
Ceiling light inoperable in 1 of 4 bathrooms located in the main activity building.
LPA requested the following documents to update the facility file: LIC500 Personnel Report. LIC610 Emergency Disaster Plan.
Deficiencies are included on the attached LIC 809D and cited from the California Code of Regulations, Title 22, and California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted and a copy of the report and appeal rights were provided to administrator Esperanza Chavez.
