The inspector’s account

01/12/2026 12:20 PM Rebecca Knight arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with house manager Cliff Taylor and staff Maraget Waldron and explained the purpose of the visit. Licensee Babe Mason was not available for the visit.

LPA Knight and staff toured the facility together to ensure the health and safety of clients in care. Areas toured include but are not limited to four (4) client rooms, common areas, one (1) bathroom, kitchen, storage areas. Staff and resident files were reviewed. All employees requiring background checks are cleared. Administrator certificate is current.

Activities and outings are planned according to the clients preferences. There is an adequate supply of toiletries for the clients. Medication is locked in a cabinet.

The facility was observed to be at a comfortable temperature. Hot water measured between 105 – 120 degrees F. Common area, bathrooms, and kitchen were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Bedding, linens, and towels for clients were observed and found to be clean and in good repair. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. Fire extinguishers fully charged inspected. Smoke detectors and carbon monoxide detectors are all operational. There are no pools/bodies of water are on premises. The facility has been conducting emergency disaster drills quarterly and fire drills monthly.

LPA requested the following documents that need to be updated in the facility file: LIC500 Personnel Report, copy of current administrator certificate.

The following deficiencies were observed during the inspection: LPA observed toilet tank lid is covered in duct tape because the lid is cracked. LPA observed cracked floor tiles in the bathroom near the base of the toilet.

California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. Appeal rights were provided, and an exit interview conducted. A copy of the report was provided to licensee Babe Mason.
