Facility Evaluation Report
08/05/2025 11:30 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with administrator Chantelle Browning. The purpose of this visit was to deliver the results of a case management related to a complaint investigation.
LPA investigated a complaint with an allegation of lack of care and supervision. This allegation was unsubstantiated. However, during the investigation LPA discovered that the facility had not submitted a report to the department for a related incident regarding an injury that was sustained by a client who lives in the facility that occurred in the facility on May 28, 2024.
Based on interviews and evidence obtained during the investigation, the preponderance of evidence standard has been met, therefore, the allegation failure to report is found to be SUBSTANTIATED. California Code of Regulations, (Title 22), is being cited on the attached LIC809D. Appeal rights were provided. Exit interview conducted and a copy of the report was provided to administrator Chantelle Browning.
