Facility Evaluation Report
07/29/2026 10:20 AM Licensing Program Analyst (LPA) Rebecca Knight arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with administrator Audra Audino ( 7013118740 exp. 01/18/2027 ) and explained the purpose of the visit.
LPA Knight and the administrator toured the facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to twelve (12) resident rooms, common areas, nine (9) bathrooms, kitchen, storage areas and back yard. Staff and resident files were reviewed. All employees requiring background checks are cleared. Administrator certificate is current.
There is a schedule of recreational activities planned for the residents and activities are planned according to resident preferences. Medication is secured. Medications were reviewed.
The facility was observed to be at a comfortable temperature. Common area, kitchen and bathrooms were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Bedding, linens, and towels for residents were observed and found to be clean and in good repair. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. Fire extinguishers fully charged and inspected. Smoke detectors are hard wired to the fire department and inspected annually by fire marshal, last inspection was conducted in July 2026. Additionally the alarm system is inspected annually by Hue & Cry Security Systems. There are no pools/bodies of water are on premises. Last disaster drill was conducted in July 2026, the facility has been conducting fire drills every 4 months.
In the areas toured no immediate health, safety, or personal rights violations were observed. No deficiencies are being cited as a result of today’s inspection.
Exit interview was conducted and a copy of the report was provided.
