The inspector’s account

On May 27, 2026, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a Required Annual Inspection. LPA met with Cole Militano, Program Manager, and explained the purpose of the visit. During the inspection, there were 23 clients and eight (8) staff providing direct care. LPA observed clients participating in arts and crafts and watching movies. Seven (7) clients were on an outing with staff.

LPA and Program Manager toured the facility together to ensure the health and safety of clients in the program. Areas toured include but are not limited to: (2) two classrooms, movie room, two (2) bathrooms, kitche and storage areas.. All areas observed were found to be clean and in good repair. All walkways were clear of obstructions. LPA observed the facility to be at a comfortable temperature. There were various activities available for client recreation including games, cards, movies, and a plethora of art supplies. Clients sign up for the activities of their choice each day.

All clients bring their lunches from home. LPA observed all medications, sharps, and cleaning supplies to be locked away and inaccessible to clients in care.

LPA observed (2) two fire extinguishers which were last inspected in April 2026. Smoke detectors and carbon monoxide detectors were observed and found to be in working condition. The facility is conducting emergency disaster drills with the last drill being documented in April 2026. LPA observed a complete first aid kit ready for use.

LPA reviewed a total of four (4) client files and four (4) staff files which contained all the required documentation. All staff are fingerprint cleared and associated to the facility.

No deficiencies are being cited as a result of this inspection and the facility is in compliance. Exit interview conducted. A copy of this report was provided to Cole Militano, Program Manager, via email
